County Nomination Forms

Each year, the Thurston and Snohomish-King County CFD Committees come together to honor local volunteers in their efforts on the CFD Annual Giving Campaign. These awards are graded by each committee. Recognition events will be held in March.

Don't see your county? Not to worry! Please use the statewide nomination form below to recognize the volunteer(s) or leader(s) in your agency!

Statewide Nomination Forms

Nominate your best volunteers and leaders in one of three categories for the CFD statewide awards. Executive Sponsors, Campaign Leaders and Local Coordinators from around the state will all be honored at local events. Remember, you can nominate the same volunteer or leader for statewide and local awards. Nominations close on Jan. 17, 2019.
